  • Key in UAN and Captcha: In the specified fields, fill in your valid UAN number and the captcha code shown in the image to verify you’re not a robot.

Furnish the essential details before filing your request. At this stage, you will also receive the option to change your phone number. If you prefer a new phone number, enter the new particulars to proceed.
